This Permit to Dig (PTD) form is completed before breaking ground and authorized by the site supervisor. It captures project details, confirms underground services have been identified, and verifies that risk assessments, method statements, and safe excavation practices are in place. It records CAT use and calibration, competent operators, and operatives’ briefings. It also includes completion checks for reinstatement, site safety, and permit cancellation or closure. Valid for one day, it helps align with HSG47 and safe excavation requirements.
