This NEP HAVS Assessment template helps teams evaluate and control hand arm vibration risks on site. Capture site and assessor details, list workers exposed, and record vibrating tools and trigger times. Calculate exposure points using vibration magnitude and duration to compare against EAV and ELV thresholds. Document control measures such as time limits, job rotation, training, tool repair or replacement, and elimination where possible. Add recommendations and obtain team leader sign off to support compliance and reduce the risk of HAVS.
