This HAVS assessment template helps teams identify and control hand arm vibration risks during tasks that involve vibrating tools and equipment. Record exposed workers, list tools and vibration sources, estimate trigger time, and calculate exposure points using a ready reckoner aligned to EAV and ELV thresholds. Plan controls such as time limits, job rotation, training, repairs, or tool replacement to reduce exposure.
