Use this HSE Observations and Near Misses Inspection form to document what was seen, where it occurred, and the type of near miss. Select hazard categories across physical, chemical, biological, and ergonomic risks, including items such as electrical shock, rotating or moving equipment, work at heights, confined spaces, radiation, dust and fumes, and poor ergonomics. Rate severity and likelihood to calculate risk, note corrective actions, and improve safety performance across projects, plants, and field operations.
