Use this hazard reporting template to log positive HSE observations, unsafe acts or conditions, and near-miss events in the field. Capture detailed hazard descriptions, photos, and locations; assess consequence and likelihood to produce a risk rating; and document corrective actions with evidence. Covers common categories including housekeeping, fire and explosion hazards, machinery, hazardous substances, electrical, environmental, slips, trips and falls, PPE, scaffolding, barriers and signs, ambient conditions, working at heights, dropped objects, lifting equipment, pressure testing, permits to work, confined spaces, noise, vibration, radiation, access and walkways, traffic, manual handling, and other hazards. Assign actions to contractors or HSE reps and track follow-up to improve safety performance.
