Initial Strike Report
This Initial Strike Report template helps construction teams document utility or service strikes on site with speed and accuracy. Capture key information including subcontractor and site lead, operatives involved, nature of works, SSRA completion, detailed account of events leading to the strike, and immediate actions taken. Record whether injuries occurred, utilities affected, utility plan availability, surface type, service depth, and presence of marker tapes or tiles. Log calibration dates for CAT and Genny, confirm above‑ground service markings, and note any mechanical excavation within 500 mm of known services. Include utility report reference numbers and attach photos to support investigation and corrective actions.
