Downloads: 20+

This hazard reporting form helps teams record and communicate hazards found on construction sites. Capture key details including a title page, client name, site address, date and time, and comprehensive hazard information. Add a photo of the hazard, describe the risk, specify the exact location, and include the reporter’s name and signature. Once completed, share the report with a supervisor or site manager to prompt timely corrective action. This template is provided as a general example from the SafetyCulture Public Library and should be reviewed for suitability and compliance with your workplace and jurisdiction.
