This Field Level Hazard Assessment (FLHA) checklist helps crews identify and control job-specific hazards before work starts. It prompts users to review PPE, muster points, and SDS where needed, then evaluate hazards across key areas: environmental (weather, dust, fumes, noise, spills, nearby workers), ergonomic (repetitive motion, tight spaces, heavy or awkward lifts), access and egress (ladders, scaffolds, aerial lifts, slips and trips, confined spaces), overhead (power lines, falling objects, hoisting), fall protection and machine safeguards, rigging and hoisting, personal limitations and training, powered mobile equipment, and chemical or biological exposures. The form guides users to record risks, controls, and rate severity and probability. It also includes lockout/tagout checks and space for worker sign-on to confirm review before commencing work.
