Use this scaffold inspection form to assess safety and compliance for roofing and construction projects. The checklist covers tag systems, purpose and duty loading, scaffold stability, distance to work face, guardrails and mid-rails, internal fall protection, face and transverse bracing, platform width and gaps, access and ladders, ties and rakers, and worker training. Scaffold Inspection Form for PCBU
This scaffold inspection form helps PCBUs verify scaffold safety and compliance. Assess stability, access, bracing, platform width, ladder security, ties and rakers, and internal fall protection. Confirm tag status and duty loading at every entry, ensure inspections are completed by a certified scaffolder, and follow manufacturer specifications and WorkSafe requirements. Document hazards, corrective actions, photos, and required notifications to the foreman. Remove tags and disable access if the scaffold is not safe until issues are resolved and the tag reads SAFE.
