This evaluation checklist helps verify an authorized end user’s fall protection competency through a single observed event. It covers training compliance (ANSI Z359 or relevant provincial standards), hazard identification, correct use of fall protection components (anchor points, body support, connectors, deceleration, arrest and restraint), and thorough equipment inspections. It also guides proper donning, doffing, adjusting, cleaning, maintenance, and storage of full body harnesses. The checklist confirms rescue readiness and ability to describe self‑rescue and assisted rescue plans. Results indicate pass/fail with space for comments, and signatures from the employee and competent person.
