This post-contract evaluation checklist helps organizations assess contractor performance after work completion. It guides reviewers to verify on-time, on-budget delivery; risk management and documentation (permits to work for hot work, work at height, and live electrical; JSAs); PPE compliance; incident and near-miss reporting; adherence to site rules; and identification of new hazards. It also checks inductions, licenses, and training such as asbestos removal, forklift operation, heavy equipment, confined spaces, working at heights, welding, and electrician registration. Use it to rate performance, capture comments, and assign remedial actions for continuous improvement.
