Use this Field Contractor Audit Template to evaluate contractor and subcontractor performance across QHSE, site safety, and quality. The checklist covers policy communication, roles and responsibilities, management commitment, and contractor control. Assess incident reporting, plant and equipment maintenance, inductions, permits, scaffolding, lifting operations, traffic management, and PPE. Capture environmental controls including spill response, pollution prevention, waste and water management, and COSHH compliance. Review quality processes such as document control, inspections, non compliance reporting, and calibration. Include mental health and psychosocial risk safeguards, add notes and photos for evidence, and complete sign off for auditee and auditor.
