Use this driver assessment instrument to evaluate heavy vehicle operators across LR to MC license classes. The checklist covers applicant and vehicle details, pre trip safety checks, control identification, engine start and stop, safe entry and exit, seating and mirror setup, steering, braking, indicating, gear selection, and load restraint. On road skills include kerb pulls, intersections, lane position, observation and cushion distance, anticipation, speed control, blind spots, roundabouts, lane selection, safe gaps, and traffic management. It also assesses hill starts, reversing, U turns for larger trucks, coupling and uncoupling for articulated vehicles, curves and bends, highway efficiency, correct gearing on hills, and breakdown procedures. Includes sections for comments, remedial actions, and signatures.
