This MHE Evaluation checklist supports daily assessments of material handling equipment operators. Record who is being evaluated, date, location, and MHE type or fleet number. Confirm pre-use checks with an attached photo. Observe safe operation according to training, including seatbelt use, maintaining safe distances from pedestrians and other MHE, and awareness of hazards and work areas. Note any corrective actions required, document toolbox talk advice if given, and capture operator and evaluator names and signatures.
