Use this forklift operator competency audit checklist to assess skills, safety behaviors, and compliance across warehouse and yard operations. Sections cover pre-shift inspections, operator observation, steering and control use, load capacity and stability, safe maneuvering and transport, stacking and de‑stacking, parking, refueling or charging (including propane handling and PPE), and trailer loading/unloading. Include sign-offs for instructor and operator. Recommended for weekly use; operators scoring below 80% should be retrained to close gaps and improve workplace safety.
