This FLT evaluation template helps supervisors assess forklift truck operators against critical safety and performance criteria. It covers pre operation checks of equipment, work area and load, correct fork positioning and load balance, safe approach, travel and reversing practices, horn use at intersections, cornering and inclines, and minimum separation from pedestrians. Post operation shutdown steps include lowering forks, setting controls to neutral, applying the parking brake, and securing the vehicle. Record competence level, provide advice, and list any additional control measures with timeframes.
