This Collapsed Pallet Locator checklist helps logistics, warehousing, and transport teams document and investigate pallet collapse incidents. Capture truck and site details, driver name, delivery docket, how pallets were loaded, maximum and actual pallet counts, and when and where the issue was discovered.
