ShopperTrak Installation Sign-Off Sheet

This downloadable sign-off sheet supports ShopperTrak installations, service, and maintenance in retail environments. Record site and technician details, check-in and check-out times, and technology used such as Brickstream, Gazelle or PDC, Orbit, and WiFi. Capture device IDs and names, validation times and counts, door coverage, door swing, exact mounting heights, total cable runs, terminations to ST600, switch, PDC, or patch panel with labeling, and any unused equipment left onsite. Document work performed with photo prompts, reasons for unsuccessful visits, and recap notes. The completion section confirms remaining tasks, demonstration to store contact, validation results, and includes customer and technician signatures with optional request for a digital copy. Use this template to standardize store rollouts and service visits.
