Dit ongevallenrapport is een praktisch sjabloon om incidenten op de werkplek consequent te registreren en af te handelen. Het omvat velden voor slachtoffer- en projectgegevens, plaats en datum van het ongeval, letsellocaties, EHBO of ziekenhuisopname, en eventuele materiële schade. Beschrijf het ongeval, het werk dat werd uitgevoerd en de toedracht volgens leidinggevende en betrokkene. Analyseer directe en indirecte oorzaken zoals onvoldoende instructie, toezicht of onjuiste werkmethode. Leg meldingen vast aan loonadministratie, Inspectie SZW en verzekering. Werk met verbetervoorstellen, technische maatregelen en definitieve besluiten om herhaling te voorkomen en de veiligheid structureel te verbeteren.
