This D and S IFCA form enables employees to report any workplace accident, incident or near miss. It captures who was involved, when and where it happened, details of events and causes, injuries sustained, damage to assets, witnesses, and consent for sharing information with relevant bodies to meet legal duties such as RIDDOR 2013. The process supports existing Health and Safety procedures and risk assessments available on the company server and members area. Submitting via SafetyCulture does not replace the organisation’s Accident Record system; if a digital report cannot be completed, staff must complete an Accident Book on return to the office. Where needed, the Health and Safety duty holder may investigate the event.
