This Animal Surrender Form helps councils, animal control, and shelters capture complete details when an animal is surrendered. It records officer names, CS number, animal identification (species, breed, sex, color, microchip, registration tag), litter information, photographs, area collected, and behavior or surrender reasons. It also gathers the surrendering person’s name, address, contact number, ID photo, and signed declaration acknowledging assessment, rehoming, or processing in line with council policy and the Domestic Animals Act 1994. Designed for accurate impound records and streamlined rehoming decisions.
