Use this guide to manage High Potential Incident notifications for drill and blast activities at mine sites. It outlines immediate scene control, internal notifications to the Department Manager and SSE, and when to notify external stakeholders such as the Mines Inspectorate and Explosives Inspector. Gather day-of-event data including equipment prestarts, photos, CCTV, weather conditions, statements, and telemetry. Complete and submit Form 1A within 24 hours and, for explosives events, the EIN within 48 hours. Track and manage the investigation via Fiori, then submit Form 5A upon investigation completion. The guide emphasizes alignment across forms, accurate timekeeping, and site-wide communication of investigation briefs.
