Use this Welding Surveillance Report template to document welding activities and verify compliance with the applicable welding procedure specification. Record weld identification and procedure details, welder name, preheat and interpass temperatures, shielding gas and flow rate, consumables and batch numbers, and weather protection. Calculate heat input using amperage, voltage, run length, time, and process efficiency, then confirm cleaning, weld profile, back gouging, and distortion control.
