This Power Tee service and maintenance record form helps engineers and range managers document each visit and mat change across all bays. Capture client and site details, engineer name, control panel color and version, and the reason for visit including annual services, call out repair, or removal. Record per-bay work such as cleaning and greasing the ball engine, repairing the frame assembly, rotating or repairing mats, checking spares, cleaning hopper liners and control panels, and retraining staff, with photos before and after. Track service status, items for owner attention, quantities of ball engines and control panels taken for repair, additional actions required, and customer sign off. Log all parts used with quantities to maintain accurate maintenance history and streamline future servicing.
