Rev: 02 - 20/05/2016 This form must be completed as part of the Preventative maintenance schedule (PM) monthly inspections. The parts to be examined for defects must be ticked off, signed and dated. Any defects observed must be reported to your Operations Manager or SHEQ Rep for further actions.
