Plant and Equipment Risk Assessment Checklist

This Plant and Equipment Risk Assessment template helps teams document assessment details, plant function and location, conditions of use, and incident history. It guides users through a structured hazard review covering entanglement, crushing and striking, unexpected movement, inability to stop, tipping, moving parts, ejected work pieces, sharp or flying objects, shearing, friction and burns, and other mechanical risks. Electrical hazards such as live conductors, overloads, damaged leads or switches, water exposure, and isolation procedures are included, along with noise and vibration risks. For each hazard, users capture risk events, likelihood, consequences, and control measures, and can attach photos to support findings.
