This checklist helps supervisors inspect emergency showers and eyewash units for AFI 91-203 compliance. It covers placement within 10 seconds of hazards, separation from electrical circuits, potable water supply, comfortable water temperature, freeze and heat protection, clear signage and lighting, hands-free valve activation, weekly activation tests, monthly inspections, eyewash bottle maintenance, training, alarms, and safeguards when units are inoperable.

This checklist helps verify compliance with AFI 91-203 and related eyewash and emergency shower standards. It covers location and accessibility (within 10 seconds), separation from electrical hazards, potable and tempered water requirements, protection from freezing and heat, clear signage and lighting, valve actuation and hands-free flow, alarm functionality, weekly activation tests, monthly inspections, and pre-use testing in infrequently occupied areas. It also includes eyewash flow control to both eyes, maintenance of self-contained units and bottles, labeling and expiration tracking, and training requirements.
