Use this fiber optic FAC checklist to assess manholes and joints on Zoom Fibre projects. Record site details, contractor and ZF representatives, region, suburb, cluster, and block. Capture manhole number, distance, number of joints, and the exact location if not found. Verify slack sufficiency and fiber work quality; note damaged locks or manholes, need for 7-way duct trimming, broken slack brackets, end-caps, filla foam, SM, housekeeping, and any required reinstatements or retaining walls. Include photos and obtain contractor, ZF, and witness sign-offs with dates.
