This Hazard Identification Form (FPB) is designed for Four Points Brisbane to capture workplace hazards across hotel operations. Users can describe hazards, select hazard types (manual tasks, electricity, equipment, chemicals, temperature, noise, radiation, biological, psychosocial), and explain context. The form guides risk assessment with consequence, likelihood, and risk rating, then records immediate controls with supporting photos. It includes long-term recommendations, post-control risk reassessment, and signatures for the person raising the hazard and management comments, with optional follow-up and formal risk assessment requests.
