Utility Pole Excavation Site Audit

This utility pole excavation site audit form helps supervisors verify safe setup and execution of pole hole digging. The checklist covers pre-start documentation such as JSA review and other site permits, confirmation of mandatory PPE, mark-outs and locates, and protection of the public.
