This Terms of Reference checklist supports field verification of water supply schemes. It captures project location details (site name, Wereda, Kushet, Tabia), GPS and well ID, and notes project type and construction or rehabilitation status. Inspect security and operations (fencing, locks, downtime, breakdown type, current beneficiaries). Evaluate institutional readiness and WASHCO governance, including presence of a guard, bank account and deposits, user fees, bylaws, member training, and gender composition. Record water quality observations (taste, color, issues) and sanitation conditions such as drainage and standing water. Document any supplies provided to the community and attach photos including wide views and plaques.
