This Staff Field Assessment checklist is designed for utilities field operations to evaluate safe work practices and performance. It covers verification of competencies and ID cards (e.g., SHEA/CSCS), pre-task risk assessments, PPE use, plan and drawing reviews, locating third-party services, trench support and safe excavation, barriers and signage, correct machine selection, and manual handling precautions. It includes electrical safety checks (volt stick, 230v/110v condition and test evidence, personal RCD), equipment calibration (multimeter, gas detector, CAT), asbestos precautions, working at height and access equipment inspections, fall restraint/arrest, confined space entry, high-pressure water jetting, lining/CCTV, waste handling and site reinstatement, paperwork and customer handover, incident reporting, vehicle condition, availability of tools and HS&E documents, spillage kits, adherence to standards, and overall safety attitude.
