This DIAC Incident Report template helps teams document environmental and safety incidents in detail. Capture initial notifications, DIAC name, incident description and location, and list any substances involved. Record agencies notified or involved—such as Fire & Rescue NSW, Hazmat, NSW Police, Council, WorkCover, and NSW Health—and note whether calls are incoming or outgoing with times and contact names. Classify incidents from Level 0 to Level 3, hand over to an Incident Coordinator, and maintain a complete EPA Communications Log to support regulatory compliance and rapid response.
