Ez az űrlap a Besen falitöltő regisztrációjához és az 1 év extra garancia igényléséhez szükséges adatok bekérésére szolgál. A kitöltés során fel kell tölteni a készülék SN számáról készült fotót, megadni a telepítés pontos címét és dátumát, az installáló cég vagy egyéni vállalkozó nevét, továbbá fotókat csatolni az üzembe helyezett készülékről és környezetéről, az RCD-ről és a kismegszakítóról.
