Zekelman Aerial Work Platform Practical Evaluation

This practical evaluation template is designed to assess aerial work platform operators after theory and hands on training. It covers pre operational requirements such as locating inspection paperwork, reporting hazards, and confirming personal protective equipment including harness and lanyard checks. It guides evaluators through area inspections, emergency lowering procedures, control familiarization, safe driving and steering, smooth elevation and lowering, and awareness of overhead and ground hazards. The final section verifies safe parking, exit technique with three points of contact, and gathers evaluator comments and required signatures.

Zekelman EWP Practical Evaluation
This practical evaluation checklist helps assess elevated work platform operators on the competencies taught in theory and on the job training. It covers pre-operational requirements such as correct PPE use, safety harness inspection, and completion of pre-use checks. Visual checks include guardrails and deck extensions, manuals, wheels and tires, boom or scissor arms, fluid levels, guards and decals, capacity plate, and anchorages. Function checks verify alarms and warning devices, controls at both stations, outriggers and stabilizers, emergency and auxiliary controls, direction and speed, brakes, and steering. Basic operating procedures assess smooth control, safe travel height and speed, line of sight, awareness of clearances, operating only in approved areas, positioning before elevating, maintaining safe distances from hazards, checking below before lowering, and safe parking. General safety covers planned path, pedestrian interaction, keeping within guardrails, charging competency, and three points of contact. Includes evaluator comments and signatures.
