This Site Safety Tour Form helps supervisors and safety leaders assess on-site conditions and behaviors. It covers visible behaviors on arrival, workplace risks such as contact with moving plant, struck-by and caught-between hazards, extremes of temperature, exposure to hazardous substances, working at heights, slips and trips, electrical contact, and manual handling risks. It includes checks for procedures and housekeeping, staff identification and competencies, toolbox talks and safety briefings, waste management, and clarity of instructions. Tools and equipment checks include correct tool use, vibration exposure, adequate lighting, proximity to plant, tool condition, noise exposure, spill kits, and calibration status. PPE suitability, condition, availability, and contingencies are reviewed, along with site welfare facilities such as cleanliness, proximity, and availability of soap and water.
