This trench box inspection checklist helps crews document the condition and compliance of trench shields before use. Capture serial numbers, box type and size, wall thickness, and spreader sizes. Inspect wall and end beam condition for punctures, scrapes, dents, waffling, deflection, and critical-area damage. Review spreader receivers, stacking lugs, top/bottom beams, and spreader bars for cracks, oval holes, bends, and missing or incorrect pins/keepers. Attach photos, note required repairs, and set disposition (in service, out of service, repair, derate, or remove). Includes fields for tabulated data availability, comments, and competent person signature with inspection date to support OSHA excavation safety requirements.
