Tree Worker Safety Observer Performance Form

This checklist helps evaluate the performance of a tree worker safety observer. It covers arrival readiness, approach and communication with the crew, and verification of JSA, ERP, and required documentation. Observation skills include PPE use, aerial rescue kit and fire tool placement, tie in points for tree and aerial lift work, two points of attachment when using cutting tools, and correct use of handsaw, pruner, and chainsaw. It checks spotter positioning, alignment of JSA with jobsite conditions, and adherence to company policies and safe practices aligned with ANSI Z133 2017. Conflict resolution criteria include stopping work when needed, communicating hazards, referencing policies and procedures, demonstrating correct techniques, documenting findings and training, assigning action items, and notifying supervisors.
