Southern USI Risk Assessment for Station Immersion 2022

This risk assessment template supports station immersion works and S.A.V installation. It guides teams to review SWMS, sign in at security, plan public safety controls, confirm surface type, and manage wet areas. It captures chemical use, ladder type and placement, EWP type and ticketed operator, and documents hazards with corresponding controls. The form includes location details and sign-offs from supervisor, leading hand, and up to six installers to verify understanding and compliance.
