Service Damage Initial Report and Investigation

This construction checklist helps teams record and investigate damage to services on site. Log the site address and location, date and time, and what was damaged, including details for electricity (overhead or buried, voltage, ducted), gas (pressure and pipe diameter), telecoms BT (buried or overhead, ducted, fiber), and water (pressure and pipe diameter). Capture what activity caused the damage, any injuries, who was supervising, machinery or tools involved, and whether service record drawings were available. Document who the incident was reported to and when, and note immediate risk controls such as isolating the area to protect people and property.
