Safety Awareness: Incident and Near Miss Reporting

This safety awareness checklist helps teams report accidents, incidents, safety observations, and near misses in a consistent way. It prompts who to report to, why reporting matters, and whether first aid is required. It reminds workers to complete accident forms, participate in incident investigations, and attend a toolbox talk or investigation meeting before returning to work. The template includes fields for site, date, preparer, auditor and auditee names, and space for advice provided, supporting a stronger safety culture and continuous improvement.
