Use this Working at Heights risk assessment checklist to identify hazards and assess risks when accessing elevated areas such as roofs, trucks, trailers, tanks, walkways, silos, and platforms. Document surface conditions, weather impacts, access and egress methods, falling object risks, and potential electrical exposures. Record existing controls like guardrails, scaffolds, caged ladders, EWP, anchor points, and catch platforms; verify their condition and inspection status. Rate current risk using the matrix, plan corrective actions, and capture residual risk, comments, assignees, and signatures. Support safer supervision and compliance for work at height.
