Plantilla de reporte para Seguridad y Salud en el Trabajo que permite documentar incidentes, actos y condiciones subestándar. Incluye campos para ciudad, fecha, hora y lugar del evento, así como datos del trabajador y el tipo de novedad: actos y condiciones inseguras, incidente o accidente de trabajo, incidente o accidente ambiental y condiciones de salud. Facilita la descripción detallada de la novedad, causas, parte del cuerpo afectada, tipo de lesión, equipo involucrado, impactos ambientales y evidencia. Contempla la acción tomada, responsables, firmas del accidentado y del coordinador SSTA, y la posibilidad de adjuntar medios. Útil para estandarizar el registro y el seguimiento de incidentes en el lugar de trabajo.
