Esta plantilla en español permite evaluar riesgos de accidente en tareas laborales. Incluye campos para describir la tarea, el proceso y el lugar, identificar peligros (mecanicos, quimicos, ergonomicos, biologicos, electricos, excavacion, caidas, incendio, ruido, radiacion, temperatura, visibilidad y mas) y valorar el riesgo antes y despues de controles. Contempla niveles de deficiencia, exposicion, probabilidad y consecuencia para obtener el nivel de riesgo e indicar la intervencion requerida. Permite registrar controles tecnicos y administrativos, uso de EPI, criterios de aceptabilidad, numero de expuestos, requisitos legales y medidas de intervencion (eliminacion, sustitucion, ingenieria, senalizacion y EPIs). Incluye cuestionario de chequeo de herramientas y recomendaciones, y finaliza con firma del inspector.
