This OSHA requested items and information checklist helps teams organize materials and details commonly asked for during complaints, incidents, or inspections. It covers client and site contacts, complaint type, incident date and time, and assigned OSHA officer. Track the status of key documents such as site-specific health and safety programs, toolbox talks and safety meetings, and employee counts. Capture company legal name, ownership, Federal Tax ID, NAICS code, corporate and regional addresses, and official project information. Record union details, whether a hazard still exists, injury notes, notification timing, subcontractor information, injured party details, and witness statements. Use it to centralize requests, note what has been supplied or is pending, and keep communications organized.
