This Ladder Safety Completion Report Card template helps trainers evaluate a new hire or technician’s ladder knowledge and safe work practices. It covers PPE use, pre use ladder inspections, proper setup at a 75 degree angle, stability checks, extending three rungs above the support point, tying off, rooftop transition, and use of 3 points of contact while climbing. It also verifies handling and transport of ladders, rooftop access, and any fear of heights, and provides space for comments and signatures from the technician and Training Team member to document completion.

Ladder Certification Checklist
This ladder certification checklist helps assess and document employee competency in safe ladder use. It covers pre-use inspections for rungs, rails, and feet, correct setup including a 4 to 1 angle, securing extension ladders and extending above the work surface, and proper A frame use without leaning. It reinforces stabilizer installation, using two hands while climbing, and the belt buckle rule to avoid overreaching.
