Use this Energized Electrical Work Permit to plan and authorize live electrical tasks when de-energizing is not feasible. The template guides teams through job details, communication methods, insulating or covering energized lines, required platforms, second-person and CPR or first aid requirements, LOTO verification, hazard identification, arc flash and arc blast assessment, people at risk, approach boundaries, PPE category selection, controls, and a 5 x 5 risk matrix. Add photos, complete a JSA with steps, hazards, PPE inspection, and actions, then obtain approvals. Close out once work is complete and equipment is returned to normal.
