This confined space initial determination checklist helps employers and assessors identify and document confined spaces and decide if worker entry is required. It guides you through vessel and space configurations, access and egress points, and whether spaces are simple or complex. It prompts hazard assessment, development of controls, lockout and tagout, gas testing, entry procedures, and rescue planning.
