Capture site information, client and dates, foundation and drawing numbers, and mix design with batch ticket references. Record excavation dimensions and conditions, soils or rock encountered, and base preparation. Log reinforcing steel details including N/S and E/W mat sizes, quantities, spacing, number of mats, vertical dowels, L-hook and U bars, and any additional rebar. Verify anchor bolts (embedded or drilled) and proper projection. Track concrete specifics such as actual volume, placement method (pump, chute, hopper), field-added admixtures, presence of cold joints or shear keys, and top-of-concrete elevation. Include comments and collect required approvals with company, signature, and date for subcontractor, contractor, and owner representative/testing/inspector.
