This Strike Report Investigation template helps construction teams document and analyze utility strikes during excavation. Capture utility details, location, and depth, record causes and site conditions, and list all personnel involved. Log CAT and Genny serial numbers and calibration dates, verify that equipment functioned correctly, and confirm PPE compliance. Include photos of utility plans, risk assessments, damaged assets, depth measurements, site mark ups, permit boards, and equipment.
